Waiver of Liability
Perfect Technique Sports Analysis, its respective officers, shareholders, directors, coaches are not liable for any claims, losses or damages on account of injury, including death or damage to property or person. With participation in athletic activities a person assumes the risk of damage to personal property, risk of severe injury, including permanent disability or death. Perfect Technique Sports Analysis has no liablity, before, during or after a client has contracted with us and used our suggestions for improvement. Further, Perfect Technique Sports Analysis is not liable for any other risks that are not known or not foreseeable at this time.
I have read the above waiver and release. I understand that I have given up substantial rights by agreeing to it and I agree voluntarily.
If the participant is a minor, I am agreeing to this release on behalf of the minor and myself.
